Position Summary

Under the clinical direction of the Medical Director at the Sandia Health Center, provides oversight of the daily management of the Community Health Program. Provides a full range of public health nursing services to the community of Sandia Pueblo.  Services are provided in patients’ homes, the health clinic, senior meal site, and community center and are provided to all age groups from pre-natal through senior citizens.

Supervision Exercised

Reports to the Health & Social Services Director. 

Supervises the Community Health Representatives and Van Driver.

Major Duties And Responsibilities

  1. Plans, conducts and evaluates a comprehensive community health nursing program for the Pueblo of Sandia community.  
  2. Assesses and diagnoses needs, and sets up a plan of action to meet these needs by providing health services to families and individuals.
  3. Provides primary care for acute and chronic health problems in collaboration with medical clinic staff, and homecare program.
  4. Directs all activities of medical and safety Community Health Representatives (CHRs) and van driver, including organization and documentation of their activities.
  5. Teaches recognition of early symptoms of communicable diseases and the importance of immunizations.
  6. Provides in-home healthcare to all populations from pre-natal care to geriatric care.
  7. Plans and participates in Programs for Diabetes Management and Primary Prevention.
  8. Plans and participates in Programs for Infectious Disease Control.
  9. Provides skilled nursing services to individuals, families, including measures to promote wellness, to prevent complications and to minimize disabilities.
  10. Updates immunization records and establishes a notification to patients that immunizations are required.
  11. Gives immunizations to patients who come into the clinic specifically for their vaccines and/or immunizations as prescribed by the Public Health Nurse.
  12. Records all immunizations given in the clinic onto the NMIIS statewide system as appropriate.
  13. Conducts epidemiological and community surveys, field investigations and research studies to appraise health needs.
  14. Participates in continuing quality improvement activities for the Public Health Nursing program.
  15. Maintains records and reporting systems according to Sandia Health Center policies for patients.
  16. Works with the Tribe, State and Federal Health Agencies, in planning health related activities.
  17. Performs other duties as assigned.

Minimum Qualifications, Education And Experience

Required

  1. High School Diploma, GED certification or equivalent.
  2. Bachelor’s Degree (BSN) in Nursing from an accredited college or university.                     
  3. Three (3) years’ experience in public health nursing including two (2) years supervisory experience.
  4. The following license and certification are required prior to employment start date:
  • Current RN Licensure with the NM Board of Nursing.
  • Current CPR/First Aid certification.
  1. Must submit a recent report of Tuberculosis (TB) skin test upon employment and provide a TB skin test annually.

Licensing Status

  1. Must be able to successfully pass a stringent background and character investigation in compliance with PL 101.630.
  2. Will require a post-offer, pre-employment and random drug screening.

Working Conditions

  1. Work is performed indoors.
  2. Work hours are 8:00 am to 5:00 pm Monday through Friday with some overtime and evening meetings required.
  3. Subject to hazardous materials which may cause bodily harm: smoke, common colds, influenza, dust, odors and elevated noise levels.
  4. Tasks may be performed on uneven, inclined, hard and soft carpeted floors, cement structures and surfaces.
  5. Duties may involve walking, standing for long periods of time, sitting and crouching.
  6. Specific required movements include the following: 
  • Trunk:  bend, twist, rotate, push, pull, carry.
  • Arms:  reach, carry, push, pull, lift, twist, rotate
  • Legs:  lift, push, pull, twist, rotate
  • Hands:  grasp, manipulate, bilateral coordination, eye and hand coordination, overall and finger dexterity.
